V1.0 - For 020/030's, uses no lookup table.
V1.1 - For 020/030's, uses 256K lookup table.
V1.2 - For 040's only, uses no lookup table.
V1.3 - For 040's only, uses 256K lookup table.
*ALL* video drivers are 640x512.
