
NOTE: Before submitting either this Software/Documentation Problem Form,
please check the OnLine Knowledge Base to insure that you are not submitting
a duplicate of an already known problem.  

The easiest way to use this form is to use the FILE IMPORT command or copy
the template to the description area of the Service Request, and then fill
in completely.  The goal with this form is not to duplicate the Service
Request header fields, but rather to structure and organize additional 
detail so that Microsoft Support Engineers can verify quickly.

                      2 = Functions incorrectly from stated documentation 
                      3 = Minor annoyance, cosmetic problem

3) Detailed Description/Results: Please include ALL the steps necessary to
   reproduce the problem - including sample code, if necessary.  Please use
   a simple template like "HELLO."
